Prize claims must be completed within 72 hours of an event ending unless otherwise specified. Failure to submit a prize claim within the allotted time may result in forfeiture of prize payment. All information on this form must be 100% accurate including a valid government issued photo ID which matches the name submitted on this form. Failure to provide accurate information may result in forfeiture of prize payment. It is encouraged that a captain or team owner claims the entire prize for their team but not required.
Please allow 3 business days for processing once a prize claim form has been successfully submitted. Once prize claim has been processed please make sure to check your email for an invitation to register an account on Bill.com with Unified Pro-Am Association, LLC. Please provide accurate personal and payment information. Upon successfully registering an account on Bill.com with UPA please allow 7-10 business days for payment to be disbursed.
